Senior High School Admission Requirements
The following documents are required for submission upon seeking admission to the college
- School Card (Form 138 / form 137)
- Certificate of Good Moral Character
- Certified true copy of Birth Certificate (NSO)
- Two (2) pieces of recent ID picture, 2"x2" (colored)
- One (1) piece Long Brown Envelope
Incoming Freshman Admission Requirements
The following documents are required for submission upon seeking admission to the college
- Original Report Card (Form 138)
- Certificate of Good Moral Character
- Certified True Copy of Birth Certificate (NSO)
- Two (2) pieces of recent ID picture, 2"x2" colored
- Long brown envelope
Transferee Admission Requirements
The following documents are required for submission upon seeking admission to the college
- Original copy of the official Honorable Dismissal
- Transcript of Records or Certification of Grades from the last school attended
- Certified True Copy of Birth Certificate (NSO)
- Two (2) pieces of recent ID picture, 2"x2" colored
- Certificate of Good Moral Character
- Long brown envelope
